1. Acceptance of Terms

By accessing and using the Rooted Health Online website and services, you accept and agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

2. Services Provided

Rooted Health Online provides online naturopathic health consultations and wellness programs. Our flagship service, "The Rooted Transformation," is a 6-month comprehensive health partnership program.

3. Professional Relationship

The relationship between Dr. Nicole Maxwell, NMD and clients is one of healthcare provider and patient. All consultations and recommendations are provided within the scope of naturopathic practice and are not a substitute for emergency medical care.

4. Program Commitment & Payment

• Duration: The Rooted Transformation Package is a 6-month program requiring your full commitment.
• Payment: Payment options include a one-time payment of $3,500 USD or six monthly payments of $667 USD.
• Lab Costs: The cost of lab tests is not included in the program fee and must be paid directly to lab companies.
• Supplement Costs: Access to professional-grade supplements is provided, but the cost of supplements is separate from the program fee.

5. No Refunds Policy

Due to the digital nature of the materials and the intensive 1-on-1 time reserved for each client upon enrollment, we do not offer refunds for the Rooted Transformation Package. We are committed to supporting you fully on this journey, and we ask for your full commitment in return. The complimentary 15-minute Clarity Call is specifically designed to ensure we are a perfect fit before any financial commitment is made.

6. Cancellation

If you need to cancel a scheduled appointment, please provide at least 24 hours notice. Failure to provide adequate notice may result in forfeiture of that session.

7. Confidentiality & HIPAA Compliance

We are committed to protecting your health information in accordance with HIPAA regulations. Your protected health information (PHI) will not be shared without your explicit written consent, except as required by law.

8. Insurance

Rooted Health Online does not accept insurance. We can provide you with a superbill that you may submit to your insurance for potential out-of-network reimbursement, but we cannot guarantee reimbursement.

9. Limitation of Liability

While we strive to provide the highest quality care, individual results may vary. Rooted Health Online and Dr. Nicole Maxwell, NMD are not liable for any indirect, incidental, or consequential damages arising from your use of our services.

10. Medical Emergency Disclaimer

Our services are not intended for medical emergencies. If you are experiencing a medical emergency, please call 911 or go to your nearest emergency room immediately.

11. Intellectual Property

All content, materials, and resources provided through Rooted Health Online are the intellectual property of Dr. Nicole Maxwell, NMD and may not be reproduced, distributed, or used for commercial purposes without written permission.

12. Changes to Terms

We reserve the right to update these Terms of Service at any time. Changes will be posted on this page with an updated "Last Updated" date. Continued use of our services after changes constitutes acceptance of the updated terms.

13. Governing Law

These Terms of Service are governed by the laws of the State of Washington, United States, without regard to conflict of law principles.
